Position Title: PATIENT CARE LEAD
Reports to: Manager, Clinical Health
Job Overview
Reporting to the Manager, Clinical Health, the Patient Care Lead is responsible for the oversight of the Haisla Nation Council’s in-home patient care services, ensuring the delivery of effective and compassionate support to community members who require assistance with their daily needs, ensuring their overall health and well-being.
The Patient Care Lead is responsible for supervision of the Patient Care Aides and the Homemakers.

We are about 2023+ people, with approximately 700 living in Kitamaat Village. We have lived off the land and waters of our traditional territory for thousands of years, and it remains the focus of all we do.
Duties & Responsibilities
Supervision and Support:
- Supervise and provide guidance to Patient Care Aides and Homemakers, ensuring they deliver safe and high-quality care.
- Schedule and manage patient care services based on patient needs and staff availability, ensuring timely and efficient delivery.
- Conduct regular team meetings to review care plans, address issues, and identify training and development opportunities for staff.
Patient Care Management:
- Develop, implement and monitor individualized care plans in collaboration with healthcare providers, family members, and community resources, ensuring they meet the specific needs and preferences of the patients.
- Monitor patient progress and adjust care plans as necessary.
- Ensure all patient care services comply with licensing requirements and best practices.
Communication:
- Act as a liaison between patients, families, healthcare providers, and patient care staff.
- Communicate effectively with patients and their families about care plans, changes in treatment, and other relevant information, addressing questions, concerns, and requests in a timely manner.
- Collaborate with other departments to ensure comprehensive care and access to services for all patients.
Quality Assurance:
- Conduct regular assessments of care services to ensure they meet established standards and protocols.
- Address any issues or concerns related to patient care promptly and effectively.
- Participate in meetings to review and address any issues related to patient care.
Administration:
- Assist in the development of budgets and manage resources effectively.
- Maintain accurate and up-to-date patient records, including health histories and care plans.
- Ensure proper documentation of all care activities and patient interactions.
- Prepare and submit regular reports and maintain records for compliance and quality assurance purposes.
Training and Development:
- Plan and facilitate ongoing training and professional development opportunities for Patient Care Aides and Homemakers.
- Ensure staff are up-to-date with mandatory certifications and knowledgeable about current best practices and regulatory requirements.
Skills & Qualifications
- Bachelor’s degree healthcare in Administration, Nursing, or a related field, or equivalent.
- 1 to 3 years of recent experience in in a supervisory or coordinating role within a home care, healthcare, or community services.
- Valid First Aid and CPR certification.
- Experience working for a band government or Indigenous organization.
- In-depth knowledge of patient care practices and regulatory requirements.
- Cultural sensitivity and understanding of First Nation traditions, values, and community dynamics.
- Ability to maintain confidentiality and handle sensitive information and discretion.
- Ability to establish and maintain respectful, cooperative and productive working relationships with a variety of individuals.
- Ability to work independently and as a team.
- Excellent interpersonal and communication skills, both verbal and written.
- Strong analytical and problem-solving abilities, with a focus on data-driven decision-making.
- Strong working knowledge of Microsoft Office (Excel, Word, Power Point), and various databases, search engines and outlook/email.
- Strong organizational and time management skills with an attention to detail.
- Ability to meet intensive and changing deadlines.
- Valid BC driver’s license and own vehicle is an asset.
- Satisfactory completion of criminal records check (with vulnerable sector screening if relevant).
Salary Range: $30.35/hr - $37.49/hr
